一、在线填写 COVA 表
请认真阅读下方“注意事项”,逐项据实填写,完成后下载 PDF、打印并签字。表单一经提交将无法修改,如需调整须重新填写。

证件照片上传
- 使用系统下方的 slider 缩放以调整头像大小,保证头部尺寸符合规范。
- 确保白色背景、无阴影、五官清晰;请勿佩戴有色镜片或夸张饰品。
个人基本信息
- 姓、名、性别、生日、国籍、护照号必须与护照信息页完全一致。
- 名(Given name):美国护照 Given names 包含中间名,均填入此栏;不单独填写 Middle name。
- 曾用名:如更名(含新增英文 First/Middle name),请填原中国护照姓名(拼音);未更名者留空。
- 中文姓名:中国出生(含港澳台)者请填写汉字姓名(如张三、李四),其他人可不填。
- National ID Number:可填驾照号或护照号。
- “是否曾拥有其它国籍或居民身份”:中国出生(含港澳台)请选择“是”,并选择中国。
- 1.6H 为必填:如曾申请过中国签证,该项在线可能不显示;请在打印后的纸质表格上,手写补充中文姓名、原身份证号、原中国护照号;实在找不到可写“不记得/找不到”。
护照/旅行证件种类
大多数申请人选 普通(Ordinary)。
申请种类 / 来华事由
- 探亲 Q2:选 D「短期探望中国公民」,停留不超过 180 天。
- 签证详情(十年签证):有效期 120 月;最长停留期—旅游 90 天、探亲 180 天;次数:多次。
当前职业与最近 5 年工作经历
- 大学中非 faculty 职位请选择「公司职员 Company employee」,不要选「学者 Academic」。
- 近 5 年所有工作经历均需填写(含中国境内),按最近的在前排序;当前工作不填截止日期。
- 务必填写职位与职责(在线表单此项为可选,但领馆要求必填)。
- 若无工作或有空档,请说明平时从事的活动及收入来源;超出 5 项可另附纸张,签名并注明日期。
- 自雇:正常填写;supervisor 可填自己;如无公司名可写“自雇”;职位 Owner;职责写明业务范围。
- 退休:仍需填写近 5 年工作;如退休超过 5 年,填写最后一份工作(洛杉矶领馆明确要求)。
教育背景
- 目前仅要求最高学历;无大学经历则填高中。
- 学位与专业必须填写(高中除外)。
地址、Email、直系亲属
- 家庭住址与配偶住址请完整填写 City / State / ZIP。
- Email 建议填写(旧金山领馆必须填写)。
- 父母信息:如已去世,选“不适用”,并注明“已去世”。
- 子女信息:成年子女也须填写。长期无子女者,打印后在附加页空白处手写“无子女”并签名。
旅行信息
- 航班:可不填。
- 停留城市:探亲 Q2 为主要探亲目的地;如旅游多个城市,应全部填写(点击 +Add 添加),时间与酒店相符。
- 地址:虽然在线为可选项,建议必填(填被探望人地址或酒店地址)。
邀请方/联系人与费用承担
- 旅游签证(L):不要填写“中国境内邀请方/联系人或组织”,以免引发误解。
- 费用承担:儿童申请人请勿选“自己”;请选择父母一方。
- 担保人:一般不需要;如为儿童,可填写父母一方。
最近一年访问国家
请如实填写。若访问了可能被视为涉恐的地区/国家,领馆可能要求补充说明,请提前与我们联系以便准备。
三、申请表下载、打印与签名
- 下载与打印:COVA 完成后下载 PDF,打印全套(含《中国签证在线填表确认页》与《中华人民共和国签证申请表》)。通常共 7–8 页。
- 如不提供纸质照片,则需彩色打印确认页且上传照片已被系统接受。
- 签名规范:确认页 + 第 7 页 9.1A 处 均须申请人亲笔签名;不可电子签;勿误签 9.2E。
未成年人(18 岁以下)签名规则
- 由监护人代签,小孩无需签字(如不清楚可致电咨询)。
- 确认页:先写儿童名字,旁边签家长名字。
- 第 7 页:9.1A 处写儿童名字;页面底部 9.2E 代填人处签家长名字。
- 《申请人在美声明表》:写儿童名字,旁边签家长名字。

在线提交后无法修改,只能重新填写一份 COVA。为避免反复,建议先做我司预审。
十年多次签证一定能获批吗?
签证签发由受理领馆综合审核并最终决定。表格中仅为期望值的选择,不保证签发结果与停留天数。
一定要提供行程单和航班吗?
航班信息通常可不填;停留城市和住宿地址建议填写且与申请目的相符。
为什么必须写“职位与职责”?
尽管在线表显示为可选,但多领区实际要求必填;缺失可能导致补件或退回。

COVA Chinese Visa Online Form (Step-by-Step Guide • Print-Ready • Common Pitfalls)
Before You Start
- Valid passport (generally ≥ 6 months; enough blank visa pages).
- Digital 2x2 inch photo (white background, recent, unobstructed).
- Confirm your submission location by state (see consular table below).

ID Photo Upload
- Use the slider under the photo to adjust head size to spec.
- White background, even lighting, clear facial features; avoid tinted lenses or large accessories.
Personal Information
- Names, sex, DOB, nationality, passport number must match the passport page exactly.
- Given name(s): On U.S. passports, Given names include middle name; enter them together—don’t separate the middle name.
- Former name(s): If you changed names (incl. adding English first/middle), enter the name from your Chinese passport (pinyin). Otherwise leave blank.
- Chinese name: If born in China (incl. HK/MO/TW), provide your name in Chinese characters; others may leave blank.
- National ID Number: Driver’s license number or passport number.
- “Previously held other nationality or residence status”: If born in China (incl. HK/MO/TW), choose “Yes” and select China.
- Item 1.6H is mandatory: If you previously applied for a Chinese visa, this field may not appear online; after printing, hand‑write your Chinese name, former ID No., and former Chinese passport No. If unknown, write “Unknown/Not sure”.
Passport/Travel Document Type
Most applicants choose Ordinary.
Visa Type / Purpose of Visit
- Q2 (Family Visit): Select D “Short‑term visit to Chinese citizens”, up to 180 days.
- 10‑year multiple‑entry (when applicable): Validity 120 months; max stay—tourism 90 days, Q2 180 days; entries: multiple.
Current Occupation & Last 5 Years’ Experience
- Non‑faculty university roles: choose “Company employee”, not “Academic”.
- List all jobs in the last 5 years (incl. in China), most recent first; no end date for current job.
- Position & duties are required (even if marked optional online).
- If unemployed or with gaps, explain activities and income source; add signed addendum if >5 entries.
- Self‑employed: fill normally; supervisor = yourself; company name can be “Self‑employed”; position Owner; describe your business.
- Retired: provide last 5 years; if retired >5 years, give your last job (explicitly required by Los Angeles).
Education
- Currently only the highest degree is required; use “High School” if no college.
- Degree & major are required (except for high school).
Address, Email, Immediate Family
- Provide full City / State / ZIP for home and spouse.
- Email is recommended (and required by San Francisco).
- Parents: if deceased, select “Not applicable” and note “Deceased”.
- Children: adult children must also be listed. If married for years without children, hand‑write “No children” on the extra page after printing and sign.
Travel Information
- Flight: not required.
- Stay cities: for Q2, the main family‑visit city; for tourism, list all cities (use +Add), dates align with hotels.
- Address: optional online but recommended (host’s address or hotel address).
Inviter/Contact & Trip Cost
- Tourist (L): do not fill “Chinese inviter/contact/organization” to avoid misunderstanding.
- Who pays: for minors, do not choose “Self”; choose a parent.
- Guarantor: usually not needed; for minors, a parent is fine.
Countries Visited in the Last Year
Answer truthfully. If you visited regions that may trigger additional review, be prepared to provide extra information and contact us in advance.
3) Download, Print & Sign
- Download & print: after finishing COVA, download the PDF and print the full set (Confirmation Page + Application Form), usually 7–8 pages.
- If you won’t provide a paper photo, print the confirmation page in color and ensure the uploaded photo is accepted.
- Signatures: Confirmation Page + Page 7 Section 9.1A must be hand‑signed by the applicant; no e‑signatures; do not sign 9.2E by mistake.
Minors (Under 18) — Signing Rules
- Signed by guardian; child’s own signature not required.
- Confirmation page: write the child’s name; guardian signs next to it.
- Page 7: write child’s name at 9.1A; guardian signs at 9.2E (agent).
- “Statement of Applicant in the U.S.”: write child’s name; guardian signs next to it.

You can’t edit after online submission. You must refill a new COVA. Use our pre‑check to avoid rework.
Is a 10‑year multiple‑entry visa guaranteed?
No. Issuance and duration are decided by the accepting consulate. Your selections indicate preference, not outcome.
Do I need a flight and full itinerary?
Flights are typically optional; listing cities and lodging addresses is recommended and should match your purpose.
Why must I provide “position & duties”?
Though optional online, many consulates require it. Missing info can trigger requests for additional documents.
