中国签证材料清单与签名要求（含示意图）
适用于在美国递交的中国签证申请。请按清单逐项准备，并遵循 单面打印 与 亲笔签名 规范。
- COVA 中国签证申请表：在线填写，下载 PDF，打印后签字（签字原件，签在第 7 页 9.1A 处）。

- 护照原件（至少还有 6 个月有效期，两张空白签证页）。
- 护照姓名照片页复印件。
- 正面免冠彩照一张（6 个月内，2×2 英寸，与在线提交一致；若照片已成功上传且确认页彩色打印，可不交纸质照片）。 参见：签证照片详细要求
- 最后一次中国签证复印件及所持外国护照复印件（如适用）。
- 如旧护照上有未过期中国签证，请将旧护照原件一并寄来。

原中国公民（含港澳台）第一次申请中国签证，需提供：
a) 最后一本中国护照原件（注销后退还）+ 两份资料页复印件；
b) 入籍证明 Certificate of Naturalization 复印件；
c) 在入籍证明空白处手写标注您最后一次离境中国的日期；
d) 若已改名，请提供相关改名文件复印件；
e) 如最后一本中国护照过期或遗失，请填写一份《声明书》： 填写入口。 - 原中国公民（含港澳台）以前申请过中国签证：提交打印申请表后，请将原中国护照号码、身份证号码手写在申请表 1.6H 处。
- 邀请函 + 邀请人身份证（彩色）。多人同时申请时，每份申请均需单独附上一套。可参考： 邀请函模板。
- 商务签证（M）：需提供 雇主信 Employer Letter。
- 旅游签证（L）：无需提供机票与旅馆预订，且不要填写邀请方。
- 《申请人在美声明表》（签字原件）。
- 驾照（正面）复印件。

Chinese Visa Document Checklist & Signature Requirements
For applications submitted in the United States. Prepare each item below and follow the single‑sided print and hand‑signature rules.
- COVA Application Form: fill online, download PDF, print and sign (original ink signature at Page 7, Section 9.1A).

COVA Confirmation Page (original hand signature).
- Original passport (≥ 6 months validity; two blank visa pages).
- Copy of passport bio page.
- One front‑facing color photo (taken within 6 months, 2×2 inch, consistent with the online upload). If the online photo was accepted and the confirmation page is printed in color, a paper photo may be omitted. See: Photo Requirements
- Copy of your most recent PRC visa and the foreign passport bearing that visa (if applicable).
- If your old passport holds a valid PRC visa, send the original old passport as well.

Former PRC nationals (incl. HK/MO/TW) applying for a PRC visa for the first time must provide:
a) Your last PRC passport original (will be returned after cancellation) + two copies of the bio page;
b) Copy of your Certificate of Naturalization;
c) Hand‑write your last departure date from China on a blank area of the naturalization certificate copy;
d) If you changed your name, provide supporting documents;
e) If the last PRC passport has expired or is lost, submit the Statement form: link. - Former PRC nationals (incl. HK/MO/TW) who have previously applied for PRC visas: after printing the application, hand‑write your former PRC passport number and PRC ID number in field 1.6H.
- Invitation letter + inviter’s ID (color). If multiple applicants apply together, include a separate set for each. Templates: Invitation Letter.
- Business (M): provide an Employer Letter.
- Tourist (L): no flight/hotel bookings required; do not provide an inviter.
- “Statement of Applicant in the U.S.” (original hand signature).
- Copy of your driver’s license (front).

- Before mailing, photocopy/scan key originals (passport, previous visas) for your records.
